Records and forms: know the next step
Registration paperwork, a records transfer and a school form are different requests. Identify what you need first so the office can explain the right process.
Transferring records? Call first
For transfers into or out of the practice, its new-patient page directs families to call for a records-release form.
Find the right office contactFor a records transfer
Keep the sender and recipient details straight. Do not assume that requesting records means the receiving office already has them.
- Which release form should I use, and who needs to sign it?
- Where should the completed request be sent?
- Does the receiving office need a particular set of records?
- How can I confirm the request was received and ask about progress?
For registration paperwork
Open forms from the practice's official new-patient page, not from an old saved copy or an unofficial mirror. Ask which documents apply and how to submit them securely.
For school, sport or camp paperwork
Have the requesting organization's instructions and deadline ready when contacting the office. Ask whether a visit is needed, what the office can complete and how collection or return works. Do not assume a same-day turnaround.
